TENANCY SUSTAINMENT TEAM
- Provide continuing support to former clients who have been re-housed
- Offer a tailored package of support unique to the clients needs
- Provide continuity of care with designated support workers to help former clients adjust to life back in the community
- Four full time workers make regular home visits, assist
with budgeting and
arrangements for medical appointments - Arrange monthly social events and outings for former clients to attend
Around 45 re-housed clients are supported at any one time.
